Individual Housing Planning
OMHSAS expects that the planning process used to develop the County Housing Plan be consumer driven. Further, OMHSAS requires the involvement of consumers in developing the County Housing Plan.
One method of soliciting consumer input is through surveys and/or interviews. This section of website provides samples of assessment tools designed to collect information for both individual housing needs and county-level planning that may be required to assist individuals in meeting their housing goals.
OMHSAS Individual Housing Assessment Tool
The Housing Assessment Tool was designed to assist peer specialists, case managers, and others providing housing planning assistance to individuals with mental illnesses. This Tool reviews a number of factors to consider when looking for a place to call home.
